We strive to work with patients in order to give the best service available. We feel it is important for patients to have a clear understanding of their insurance benefits and coverage. Thus, we communicate directly with patients and their insurance companies to better define their insurance coverage and facilitate claim procedures.

 

Some insurance companies have certain requirements and restrictions. We encourage all patients to become aware of their individual plans in order to facilitate treatment.

If you have Medicare you MUST have a referral from your primary care physician or other specialist before we can treat. You may fax the referral to 312-332-0847

Your insurance may require you to have prior-authorization for therapy services. We encourage you to ask your insurance what your requirements are.
